I'm not sure how MoCA translates over cable internet since I use fiber but unless your modem supports MoCA via coax (which I think is uncommon?) you need two MoCA devices. There may also be an issue with interference between cable internet and MoCA but it's been a long time since I've read up on MoCA and any related frequencies. Without MoCA output from your modem/router, you will use one MoCA device at your Router and connect it via ethernet then you will run a coax cable from that MoCA device's coax-out and connect it to the coax cable that runs to your destination which is possibly all of your coax in your home.
